A suicide risk assessment template can be located in the Pillars All Staff Shared Documents folder. If you do not have access to this, please e-mail admin@pillarsinspires.com for access. Click the link below for a pdf version of the Suicide Risk Assessment.
You may also administer the Columbia-Suicide Severity Rating Scale (CSSR) located in the dashboard the client’s chart under Questionnaire Forms. This assessment should be reviewed immediately after completion in order to implement any safety measures that may be required based on responses.
