High Risk Situations

The intake paperwork addresses potential high-risk topics such as trauma and suicide. When helping a client register for IOP services, please SKIP the questions on trauma and suicide located in the “Trauma/Risk History” section. These questions will be addressed by the intake clinician during their intake.

If the client reports any current thoughts of suicide at any point during the phone call, follow the normal protocol for a client reporting suicidal ideation over the phone, which is consulting with clinical staff and/or referring to the hospital.
